Insurance Certificates and Requirements

Requesting Proof of University Coverage

Risk Management arranges for the issuance of certificates of insurance to provide evidence of coverage to third parties and to provide evidence of Medical Professional Liability insurance for providers.

Receiving Certificates of Insurance

Saint Louis University purchases products and services from numerous businesses annually. The University mandates proof of insurance from many vendors, with specific requirements outlined in the contract. The types and amounts of insurance needed depend on the services provided and the contractual obligations of the parties. The University reserves the right to require higher insurance limits on any contract.

Standard Minimum Insurance Requirements for Vendors and Contractors

All vendors, contractors, and service providers performing work, selling, or distributing products and services on behalf of, or on the premises of, Saint Louis University are required to maintain insurance coverage that meets the following minimum standards.

Departments should request a Certificate of Insurance (COI) from the contractor, vendor, or provider prior to the beginning of work and/or the start of the contract. A COI is a standard form issued by the insurance company evidencing the insurance information of its policyholder.

If a product or service presents an unusual or exceptional risk, as determined by Saint Louis University’s Office of Risk Management, additional insurance coverage may be required. The University reserves the right to require higher insurance limits at its discretion.

General Requirements
  • Vendors must submit evidence of insurance prior to the commencement of services or execution of any agreement.
  • Vendors must name Saint Louis University as an Additional Insured on their General Liability, Automobile Liability, and Cyber Liability policies.
  • Vendors are responsible for ensuring that all subcontractors maintain insurance that meet these same requirements.
  • Saint Louis University reserves the right to modify insurance requirements based on the nature of the risk, prior experience, coverage, or other unique circumstances.

Minimum Coverage Standards

Commercial General Liability Insurance
  • Coverage must include bodily injury, property damage, independent contractors, contractual liability, product liability, and completed operations.
  • Vendor shall name Saint Louis University as an additional insured on a primary and non-contributory basis.
  • Policy will include a waiver of subrogation.
  • Minimum limits: $1,000,000 per occurrence, $2,000,000 aggregate.
Automobile Liability Insurance
  • Required for vendors operating vehicles on university property.
  • Coverage must include owned, hired, and non-owned vehicles.
  • Vendor shall name Saint Louis University as an additional insured on a primary and non-contributory basis.
  • Policy will include a waiver of rights of recovery.
  • Minimum limit: $1,000,000 combined single limit per accident for bodily injury and property damage.
Workers' Compensation Insurance
  • Policy will include a waiver of rights of recovery, where allowable by law.
  • Workers’ Compensation/Employer’s Liability: $1,000,000 for bodily injury per accident and $1,000,000 for bodily injury by disease per employee.
Cyber/Privacy Liability Insurance
  • Required only for projects involving access to, or use of, Saint Louis University’s network, data, or digital systems.
  • Coverage against liability for data breaches, unauthorized access or other cyber-related incidents.
  • Coverage must include costs associated with notification, defense, and any fines or penalties arising from a breach event.
  • Minimum Limits: $1,000,000 per occurrence and $3,000,000 in the aggregate.

Medical Professional Liability Claims History

This form also serves as a means for medical credentialing offices, practitioners, and hospitals to request claims histories on individuals who have applied/reapplied for staff membership, clinical privileges and/or permission to provide services at a facility.
